Your Care Plan
Your Care Plan:
Welcome!
At our clinic, we believe that every patient deserves a care plan that fits their unique needs. Our goal is to help you get the best results possible, in a way that feels right for you. Here’s how we create your care plan and what you can expect.
Why Do We Use Care Plans?
A care plan is simply a roadmap for your treatment. It helps you know what to expect, how long your care might take, and what steps we’ll take together to help you feel better. Having a clear plan means you’re more likely to get the results you want, and it helps us work together as a team.
Not One-Size-Fits-All
Some clinics give every patient the exact same treatment plan, no matter what their problem is. Others make a completely new plan for every single person, which can be confusing and overwhelming. We believe the best approach is in the middle: we use our experience and your unique situation to place you in the care plan that fits you best.
Our Three-Tiered Care Plans
Most patients fit into one of three care plan “tiers” based on their condition:
Mild Care Plan
For new or minor issues, like a recent neck strain or mild discomfort.
Typical duration: a few weeks to a couple of months.
Moderate Care Plan
For ongoing or moderate problems, such as back pain that’s lasted a while, headaches, or
recurring discomfort.
recurring discomfort. Typical duration:
a couple of months to several months.
Severe Care Plan
For more serious or long-term issues, like chronic pain, nerve problems, or conditions
that have lasted for many months or years.
Typical duration: several months or more, with more frequent visits and possibly
additional therapies.
How Do We Decide
We look at several things, including:
Your condition:
How long you’ve had it, how severe it is, and how much it affects your life.
Exam findings
What we see in your tests, posture, and movement.
Your health history
Any other health issues or past injuries.
Your goals
What you want to achieve with care.
We use our clinical experience and your input to choose the plan that fits you best. If your needs
change, your plan can change too!
What If My Condition Is Unique?
Some conditions need a special approach. For example, if you have a specific issue like
frozen shoulder or a severe disc problem, we may have a unique care protocol just for that.
We’ll always explain your options and make sure you understand your plan.
Progress, Not Perfection
Our goal is to help you make real progress—whether that’s full recovery or a big
improvement in your quality of life. Even if you don’t reach 100% right away, every step
forward matters. We’re here to support you at every stage.
Your Role
We want you to feel confident and comfortable with your care plan. If you have questions
or concerns, please ask! Your understanding and commitment are key to your success.
