Important Information for Patients
Risk Disclosure:
Changes to the law now require all practitioners who manipulate the spine to warn patients of material risks. In extremely rare circumstances, some treatments of the neck may damage a blood vessel and give rise to a stroke or stroke-like symptoms (approx.1 in 5.85 million) [Neck manipulations. Halderman, et al Spine vol 24-8 1999]. Whilst this has never occurred in this practice, we are still required to warn. If any adjustments (manipulations) are required, you will be tested beforehand, as has always been our practice.
Other very slight risks include strain/injury to a ligament or disc in the neck (less than 1 in 139,000) or the low back (1 in 174,000) [Dvorak, J Man Med 1989; 4:7-16]. Chiropractic adjustments of the spine are internationally recognised as being far safer in dealing with neck and low back pain than medication and many other alternatives [A Risk Assessment of Cervical Manipulation, JPMT 1995, Manga Report, Ontario Ministry of Health 1993].
I Hereby request and consent to the performance of Chiropractic adjustments, other Chiropractic procedures and if necessary diagnostic x-rays on me by Dr Montgomery Harrison / Marsden Park Chiropractic.
I further understand and am informed that, as in all health care, there are some slight risks to treatment and do not expect the Chiropractor to be able to anticipate or explain all risks and combinations: and wish to rely on the Chiropractor to exercise judgement during the course or the procedure which the Chiropractor feels at the time based upon facts known, is in my best interest.
I further understand that Chiropractic care cannot guarantee resolution of some or all of my ailments.

Missed Appointment Policy
Our commitment is to deliver quality healthcare to all our patients promptly. No-shows, late arrivals, and cancellations disrupt not only our providers but also other patients. Please familiarise yourself with our policy regarding missed appointments.
Appointment Cancellation
When you book an appointment, you reserve a spot on our calendar that becomes unavailable to other patients. To be considerate of your fellow patients, please inform Dr Montgomery Harrison or Dr Ben Hsiung as soon as you know you cannot attend your appointment.
If you need to cancel, we require at least 6 hours’ notice. Appointments are in high demand, and your timely notice allows another patient to utilise that appointment slot.
How to Cancel Your Appointment
To cancel your appointment, please call or text us at 0426 966 170. Alternatively, you can use the “manage your appointment” link. Please note that this option closes 6 hours prior to your consultation. Failure to cancel or reschedule before this deadline will result in the full consultation fee being charged.
Late Cancellations/No-Shows
A cancellation is considered late if it occurs less than 6 hours before the scheduled time. A no-show is when a patient misses an appointment without cancelling. In both cases, we will charge 100% of the missed appointment fee plus a processing fee.
